The User Content Report screen presents a form with an email and report description for the user to submit a report.

The heading label can be easily customised to better direct the user.

Email

The email input view is a field that allows the user to leave their email.

Description

The description input view is a field that allows the user to enter the main body text of the report. This uses a UITextView rather than a UITextField under the hood.
